What is Reema Health?

Reema Health is a community-based organization that provides personalized, one-on-one support services to all State Health Plan members, including Medicare primary. Our services are available at no cost to you.

What is a community guide?

Community guides are Reema employees that live and work in the same city as you. They are experts at navigating health and social care services, community resources, and are here to provide you with the support you need, when you need it.

Frequently asked questions

Yes! Reema Health is available to all State Health Plan members. Right now, our services are available in the orange counties on the map below.
 

If your county is gray, we’re not there just yet, but we’re expanding fast and expect to be in your areas soon! 



No! Reema Health will never send you a bill or ask for payment. This program is a benefit provided through the State Health Plan at no cost to you.

You don’t have to sign up by a certain date. Reema Health is here whenever you need support.  

Most people work with their Reema community guide for about a year, but the length of time depends on your needs.

Yes! Our community guides are trained professionals who go through background checks and follow strict safety guidelines. You decide where and how you meet—if you’re not comfortable meeting at home, you can connect over the phone or in a public place.

Yes! With your consent, your community guide can collaborate with your care manager to help you with what you need. There’s no duplication of services, and working with Reema won’t affect your existing care.

Reema was announced as a new benefit on PEBAs website here: https://www.peba.sc.gov/news . Community guides are Reema employees that live and work near you. If you’re meeting a community guide in person, they will always have an official badge to confirm their identity. You can also contact BlueCross Customer Service at 803.736.1576 to confirm your community guides identity.
